Once enrolled in PACE, you are assigned to an interdisciplinary
care team (IDT) of doctors, nurses, social workers, therapists,
aides, and others. This team works with participants and their
families to develop a comprehensive care plan. They will then
coordinate all necessary appointments, fill prescriptions, and
provide transportation to and from PACE centers.
PACE centers are the hub for medical check-ups, physical /
rehabilitative therapy, meals, social interaction, and
recreational activities.*
*Participants must receive all needed healthcare services (other than emergency services), including primary care and specialist physician services, from the PACE provider or from an entity, person or company authorized by the PACE provider. Participants will be financially responsible for any unauthorized or out-of-network services.
