The easiest way to apply for PACE.

The Program of All-inclusive Care for the Elderly (PACE®) provides all needed healthcare and support so older individuals can continue to live in their own home. Funded by Medicare and Medi-Cal, most people enrolled in PACE do not pay anything. PACE Path makes it easy to apply and get enrolled in PACE.

Who is Eligible?

How do I know if I qualify?

While the state of California makes all determinations, if you fit within the above eligibility criteria, you should apply. PACE Services

Once enrolled in PACE, you are assigned to an interdisciplinary care team (IDT) of doctors, nurses, social workers, therapists, aides, and others. This team works with participants and their families to develop a comprehensive care plan. They will then coordinate all necessary appointments, fill prescriptions, and provide transportation to and from PACE centers.

PACE centers are the hub for medical check-ups, physical / rehabilitative therapy, meals, social interaction, and recreational activities.*

*Participants must receive all needed healthcare services (other than emergency services), including primary care and specialist physician services, from the PACE provider or from an entity, person or company authorized by the PACE provider. Participants will be financially responsible for any unauthorized or out-of-network services.

How much does PACE cost?

PACE is funded by Medicare and Medi-Cal. Most participants do not pay anything. There is never a co-pay, deductible, or coverage gap. However, if you have a Medi-Cal share of cost, you will pay that directly to your PACE provider.If a participant is not eligible for Medi-Cal, they can pay the Medi-Cal portion plus Medicare Part D. 99% of PACE participants are Medi-Cal eligible.If you are not already enrolled in Medi-Cal, we can help you apply.

What is the application process?

  • Simply fill out the application.
  • We will contact you with a follow-up appointment to make a level of care determination.
  • We will then submit your application to the state of California.
Once the state has made a determination, we will let you know if you have been accepted into PACE.If you have questions along the way, we are available and ready to help. Please don’t hesitate to contact us.There is no cost to apply. No application fees, and we never charge for our services.
